Daily Acts Liability Waiver

LIABILITY RELEASE: As a participant in Daily Acts’ Programs, I hereby release Daily Acts from all liability, including liability arising from theft or any act of negligence or want of ordinary care on the part of Daily Acts and/or any of its agents or employees.  

In consideration of my participation in the above stated outing sponsored Daily Acts, I waive, release, and discharge Daily Acts, its directors, officers, employees, agents, contractors, volunteers and members from any and all claims of such liability, including claims relating to injury or damage from any cause to me or my property arising out of my participation.  This release applies whether or not such injury, damage, or loss is any way connected with participation in any activities, whether it results from my own negligent acts, from acts of other participants, or any of the above mentioned Daily Acts affiliates. This agreement is binding upon my executors, heirs, and assigns.

PHOTO/VIDEO RELEASE: Unless I request otherwise (opt out), I hereby give permission for Daily Acts and parties designated by Daily Acts to photograph myself and use such photos in all forms of media for any and all promotional purposes including advertising, publicity, and all around goodness. I can request a copy of said photo(s).  If requested (opt out), photos of me will not be used.
